Prospective interns are expected to apply for internship at the LGH Neuropsychology Specialists Pre-doctoral internship by completing the following materials. All application materials must be received no later than December 1.
1. APPIC Uniform Application materials (including Professional Conduct Form, Practicum Documentation, Verification of Internship Eligibility and Readiness, etc.)
2. Curriculum vita
3. Official graduate transcripts
4. One assessment report (preferably neuropsychology related). Also, remember to remove all identifying information to protect patient confidentiality.
5. Three letters of reference (at least two must be from current supervisors)
