Medical Records Requests
Address your requests for copies of medical records to Medical Record Services at Lancaster General Hospital.

You must complete a signed authorization form to obtain a copy of your medical records. You may complete the authorization by:

• Coming to the Medical Record Services office on the first floor of the hospital at 555 North Duke Street. Hours are 8:30 a.m.-5 p.m. Monday through Friday. or
• Printing the following document:
  Authorization to Use or Disclose Health Information (English)
  Authorization to Use or Disclose Health Information (Spanish)
  (you will need Adobe Acrobat to view) and mailing or faxing it to:

Birth Certificate/Birth Certification Requests
Lancaster General does not provide copies of birth certificates. Copies can only be issued by the PA Department of Health, Division of Vital Records. Their website contains details on how to obtain a copy of a birth certificate.

Lancaster General can provide a Certification of Birth that might meet your needs. To obtain one, please follow the procedures listed in Medical Records Requests (above).
