VNA Community Care Services provides high-quality home health care allowing patients to recover, or live with an illness, in the comfort of their homes, surrounded by family and friends. By giving care, education and support, our professional staff helps patients learn how to monitor and care for themselves. Studies show patients who stay at home, or return home to recover, may get better more quickly.
For nearly 100 years, people have relied on VNA Community Care Services to provide home health care when they are too sick or unable to care for themselves but well enough to not be admitted to a hospital or nursing home. Common conditions that may require home health care include diabetes, heart and lung disease, cancer, stroke, and rehabilitation after surgery.
By working with a physician, the appropriate VNA Community Care Services’ staff of nurses, rehabilitation therapists, social workers and home health aides will tailor a home health care plan to meet the specific needs of the patient.
To be eligible for home health care services, you must be homebound, have a physician’s order for skilled care and authorization by your insurance.
